Pathfinder

Pathfinders will reach out to these individuals, offering a wide range of emotional and psychosocial support to help better the individuals with their personal circumstances, substance misuse and other complex social issues that they may face.

Pathfinder engagement will allow people to effectively connect with support teams that can help prevent re-offending, continued substance misuse and further harm and trauma. This particular work is done through our ongoing outreach and stabilisation period where we work with individuals to achieve goals set by both the individual and Pathfinder.
