Community Health Matters: Signposting and support

Community Health Matters aims to reduce feelings of isolation and loneliness experienced by people living in our rural communities.

Signposting as well as 1:1 support to enable anyone who is isolated or lonely as a result of a long term health condition, mental health difficulty or being a carer, to access activities, groups and services in the local area.
