Carers’ Long Term Condition Group

This service is for anyone caring for someone with a Long Term Condition, or for carers who are themselves managing a Long Term Condition. The aim is to support carer’s to better manage the condition by improving health and well being and providing access to information and support.

The groups are held on a monthly basis and are facilitated by our Carer Support Workers.
