Healthy Options Programme - ADAPT

Healthy Options ADAPT is a 12-week Specialist Programme for people who are newly diagnosed or require specialist support. ADAPT helps clients learn the skills, tools and coping strategies to improve their own health, wellbeing and quality of life whilst living with a long-term condition.

ADAPT is accessed via GP or Health Professional referral. Healthy Options works alongside clients to ensure that the right support is offered. Lasting 12 weeks, the programme consists of a supported self-management programme delivered by Level 4 and Level 5 Exercise Professionals.

Upon successful completion of the ADAPT programme, we offer an onward programme for graduates. Click the link to find out more about THRIVE.
