Care at Home

Glasgow City Health and Social Care Partnership (HSCP) Care Services (also known as Care at Home) provide a range of services and support to help you live independently at home for as long as you can and want to.

If you are assessed as eligible for a service, your care and support will be provided by one or more of the Care at Home teams which include:

First Response Team, who look after you when you are discharged home from hospital
Reablement Home Care Service for support to get back to doing what you could do for yourself before you were admitted to hospital or had an accident
Mainstream Home Care Service for support with personal care, taking medications and preparing food
Hot Meal Service (iCare) who provide and prepare meals directly to your home
Community Alarms (Telecare) for support with equipment that helps you stay safe at home
