Y sort It Care Connections partner with young people, schools and communities to help deliver on ‘The Promise’ - Scotland’s promise to care experienced children and young people that they will grow up loved, safe and respected.

Care Connections aims to increase the opportunities and choices for all children and young people we work alongside, maximising participants’ learning opportunities and experiences. Our approach includes measuring our impact within our communities to close the poverty-related attainment gap between care-experienced children and young people and their peers.

Our Care Connections support workers collaborate with individual children and young people, aged 8- 16 yrs, on both a 1-2-1 and in small groups who are living at home, on a Compulsory Supervision Order or are in formal Kinship Care and live in West Dunbartonshire. Together, we develop and implement a supportive pathway throughout a young person’s school life and beyond.

How does it work?

A Care Connections Support Worker will meet up with the child or young person to identify needs and interests and develop a plan to help them to achieve their goals. Support varies for each individual and examples include:

  • 1:1 Support
  • Group Sessions
  • Learning Opportunities
  • Trips and Visits
  • Free Gym and Swimming Passes
  • Support with school work.
