Community Specialist Diabetes Service Health & Social Care Partnership

Part of a local enhanced service within Inverclyde HSCP for people with Type 2 diabetes to support self-management, reduce risks to health and prevent hospital admission.

The team provides a service for those at greatest risk and provides support for complex patients locally and work with the referrer to provide treatment changes and intensification, monitoring, lifestyle management, advice and support.

Patients are assessed and treated at community clinics held at ICIL, or using attend anywhere technology for virtual clinic or where required are seen within their own home. The provision of education and training includes local structured group education for people newly diagnosed with Type 2 Diabetes.
