Community Occupational Therapy Health & Social Care Partnership

The Community Occupational Therapy Service is dedicated to promoting independence in all areas of daily life for both children and adults with physical disabilities and sensory impairments.

Our specialist Occupational Therapists are experts in moving and handling, sensory needs, and environmental assessment to support independent living. They carry out home visits to assess individual needs and, where appropriate, recommend and prescribe suitable equipment, rehabilitation programmes, techniques, and home adaptations to promote safety, accessibility, and independence.

We work closely with other health and social care professionals to ensure a coordinated approach to care and support delivery, helping individuals achieve the best possible outcomes for independent living.

The Community Occupational Therapy service can assist you with the following:-

  • Assessment for equipment / adaptations
  • Specialist Case management
